We will need your doctor's referral for any Medical Nutrition Therapy. You can find the referral form in the "For Providers" section.
Your initial appointment with the Registered Dietitian Nutritionist will last approximately 60-90 minutes. This allows you and your Registered Dietitian Nutritionist to address your concerns and determine your personalized nutrition plan.
Please bring a list of your medications, recent laboratory results (if available) and any other information you feel will be helpful.
Please help us serve you better by keeping scheduled appointments. Kindly give at least a 24 hour notice of cancellation.
A $25.00 fee may be assessed for all missed appointments.
Health claim forms and super-bill will be submitted to you upon request for possible insurance reimbursement. Please check with your insurance provider for nutrition counseling benefits in advance.
We accept cash, checks and major credit cards.
COVID19 Precautions: if you decide to book an in-person visit, we need to go through a questionnaire before your arrival, your temperature will be taken upon arrival, you need to wear a mask at all times, wash/sanitize hands upon arrival, and let us know in advance if you are experiencing : Fever or chills, Cough, Shortness of breath or difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of taste or smell, Sore throat, Congestion or runny nose, Nausea or vomiting, or Diarrhea.
